Health Quality Ontario (HQO) is an arms-length agency of the Ontario government that acts as a partner and a leader in transforming Ontario’s healthcare system so that it can deliver a better experience of care, better outcomes for Ontarians and better value for money. One aspect of its legislated mandate under the Excellent Care for All Act, 2010, is to support quality improvement activities in the healthcare system.
To support quality improvement in primary care, HQO engaged Sandbox Software Solutions to develop a secure web-based reporting system to enable primary care teams to enter their monthly data, review their progress and benchmark with colleagues participating in the same initiative. This Learning Community streamlined and improved data collection and reporting and provided HQO with access to reporting and graphing tools for analysis and evaluation.
Used by over 100 primary care teams across Ontario, the Learning Community launched in 2011 to track monthly measures and data. The system reduces dependency on spreadsheets, makes data entry easier and reduces the risk of human error typically associated with other data collection methods. To enhance the reporting tools, a comprehensive database links to the Learning Community capturing participant demographics to enable querying and segmenting.
