The Guelph Family Health Team is part of the Ontario Government’s approach to primary health care that brings providers together to co-ordinate the highest possible quality of care for patients within a community. With the focus on the community, the Family Health Team is set up based on the primary needs of the patients, with special attention to chronic disease management, disease prevention and health promotion. The Family Health Team also allows for access to other health care organizations, such as public health units and Community Care Access Centres
In order to give the clinic an online presence, the Guelph Family Health Team initiated work with Sandbox Software Solutions to develop a customized Content Management System, along with customized add-ons to enhance the website. The primary goal of this website is to present information about the team and services to staff, and current and potential patients; as well as, enhance the professionalism and credibility of the Health Team.
By visiting the website, patients can now easily find information about their FHT, their clinic’s policies, sign up for classes, and find health resources effortlessly. The website features many Web 2.0 features to engage and involve its target audience. Since developing the website, Sandbox Software has also implemented a secure staff portal that allows for easy and efficient collaboration among staff members along with a similar look and feel of the public-facing website.
